Case Manager - intake and assessment (Austin, TX)

We are growing our team at Foxglove Clear Care Services Group and adding a Case Manager whose main responsibility will be intake and assessment. We train and promote from within. What you will do • Help with applications and appointments for licensing. • Meet clients, understand their needs and build a simple plan. • Connect clients with partner agencies. • Report outcomes to funders in clear, short summaries. • Follow up regularly and update records accurately. What we are looking for • Experience in social services, casework or a related field. • Strong documentation habits. • Empathy, patience and good boundaries. Nice to have • Bilingual English and Spanish. • Experience with licensing. Working arrangement • Contract for nine months, with clear deliverables from the first week. • On site in Austin. • We will tell you what to expect at each step of the hiring process.
