Case Manager - intake and assessment (Atlanta, GA)

Summit Fern Care Services is looking for a Case Manager. The job centres on intake and assessment for family communication, and it would suit someone who likes steady, practical work. The work is personal and so is the culture. What you will do • Connect clients with partner agencies. • Meet clients, understand their needs and build a simple plan. • Follow up regularly and update records accurately. • Help with applications and appointments for family communication. • Report outcomes to funders in clear, short summaries. What we are looking for • Empathy, patience and good boundaries. • Experience in social services, casework or a related field. • Strong documentation habits. Nice to have • Experience with family communication. • Bilingual English and Spanish.
