Case Manager - intake and assessment (Atlanta, GA)

Greenridge Birch Care Services is a social-care provider serving older adults and people with disabilities around Atlanta, and we need a Case Manager to lead our intake and assessment. You would join a large organisation of about 825 people based in Atlanta. What you will do • Report outcomes to funders in clear, short summaries. • Help with applications and appointments for visit scheduling. • Meet clients, understand their needs and build a simple plan. • Follow up regularly and update records accurately. • Connect clients with partner agencies. What we are looking for • Empathy, patience and good boundaries. • Experience in social services, casework or a related field. • Strong documentation habits. Nice to have • Bilingual English and Spanish. • Experience with visit scheduling. Working arrangement • Contract for eight months, with the possibility of becoming permanent. • On site in Atlanta, with some flexibility after the first few months. • Applications are reviewed on a rolling basis.
