Case Manager - intake and assessment (Atlanta, GA)

Ironwood Linden Care Services is looking for a Case Manager. The job centres on intake and assessment for care plans, and it would suit someone who likes steady, practical work. You would join a large organisation of about 620 people based in Atlanta. What you will do • Meet clients, understand their needs and build a simple plan. • Help with applications and appointments for care plans. • Connect clients with partner agencies. • Report outcomes to funders in clear, short summaries. • Follow up regularly and update records accurately. What we are looking for • Experience in social services, casework or a related field. • Strong documentation habits. • Empathy, patience and good boundaries. Nice to have • Experience with care plans. • Bilingual English and Spanish. Working arrangement • Part-time, about 28 hours a week, afternoons. • On site in Atlanta, with some flexibility after the first few months. • We reply to every applicant.
