Case Manager - intake and assessment (Anchorage, AK)

Cedar Oak Health Group is a healthcare provider in Anchorage with clinics and support teams, and we need a Case Manager to lead our intake and assessment. We are busy and kind, and we look after each other. What you will do • Connect clients with partner agencies. • Follow up regularly and update records accurately. • Report outcomes to funders in clear, short summaries. • Meet clients, understand their needs and build a simple plan. • Help with applications and appointments for electronic records. What we are looking for • Strong documentation habits. • Experience in social services, casework or a related field. • Empathy, patience and good boundaries. Nice to have • Bilingual English and Spanish. • Experience with electronic records. Working arrangement • Full-time, on a regular day shift. • On site in Anchorage. • Applications are reviewed on a rolling basis.
