Case Manager - follow-up and outcomes (Wichita, KS)

Stonegate Vista Community Partners Group is a nonprofit in Wichita supporting families with food, housing and job help. We are hiring a Case Manager to take on follow-up and outcomes. Mission matters here, and so does looking after staff. What you will do • Report outcomes to funders in clear, short summaries. • Connect clients with partner agencies. • Meet clients, understand their needs and build a simple plan. • Follow up regularly and update records accurately. • Help with applications and appointments for benefits enrolment. What we are looking for • Empathy, patience and good boundaries. • Experience in social services, casework or a related field. • Strong documentation habits. Nice to have • Experience with benefits enrolment. • Bilingual English and Spanish. Working arrangement • Part-time, about 16 hours a week, with flexible days. • On site in Wichita, with some flexibility after the first few months. • We will tell you what to expect at each step of the hiring process.
