Case Manager - follow-up and outcomes (Wichita, KS)

We are growing our team at Stonegate West Care Services Group and adding a Case Manager whose main responsibility will be follow-up and outcomes. We train and promote from within. What you will do • Help with applications and appointments for caregiver training. • Report outcomes to funders in clear, short summaries. • Meet clients, understand their needs and build a simple plan. • Connect clients with partner agencies. • Follow up regularly and update records accurately. What we are looking for • Strong documentation habits. • Empathy, patience and good boundaries. • Experience in social services, casework or a related field. Nice to have • Experience with caregiver training. • Bilingual English and Spanish. Working arrangement • Full-time, on a regular day shift. • On site in Wichita. • We reply to every applicant.
