Case Manager - follow-up and outcomes (Wichita, KS)

Lumen Lantern Learning Group is an education provider in Wichita serving students and families, and we need a Case Manager to lead our follow-up and outcomes. We follow the school calendar and we like people who like learning. What you will do • Meet clients, understand their needs and build a simple plan. • Help with applications and appointments for student accounts. • Connect clients with partner agencies. • Follow up regularly and update records accurately. • Report outcomes to funders in clear, short summaries. What we are looking for • Strong documentation habits. • Empathy, patience and good boundaries. • Experience in social services, casework or a related field. Nice to have • Bilingual English and Spanish. • Experience with student accounts. Working arrangement • Contract for eight months, with a review at the end and a chance of extension. • On site in Wichita, with some flexibility after the first few months. • Applications are reviewed on a rolling basis.
