Case Manager - follow-up and outcomes (Wichita, KS)

Fernwood Mill Care Services Group provides in-home and community support in Wichita. We are hiring a Case Manager to take on follow-up and outcomes. You would join a small organisation of about 134 people based in Wichita. What you will do • Report outcomes to funders in clear, short summaries. • Follow up regularly and update records accurately. • Connect clients with partner agencies. • Help with applications and appointments for visit scheduling. • Meet clients, understand their needs and build a simple plan. What we are looking for • Experience in social services, casework or a related field. • Empathy, patience and good boundaries. • Strong documentation habits. Nice to have • Bilingual English and Spanish. • Experience with visit scheduling. Working arrangement • Contract for six months, with clear deliverables from the first week. • On site in Wichita. • We reply to every applicant.
