Case Manager - follow-up and outcomes (Wichita, KS)

Rivermark Cobalt Property Management looks after rental homes across the Wichita area, and we need a Case Manager to lead our follow-up and outcomes. You would join a small organisation of about 21 people based in Wichita. What you will do • Meet clients, understand their needs and build a simple plan. • Follow up regularly and update records accurately. • Help with applications and appointments for resident events. • Report outcomes to funders in clear, short summaries. • Connect clients with partner agencies. What we are looking for • Strong documentation habits. • Experience in social services, casework or a related field. • Empathy, patience and good boundaries. Nice to have • Bilingual English and Spanish. • Experience with resident events. Working arrangement • Full-time, with a steady weekday schedule. • On site in Wichita. • Applications are reviewed on a rolling basis.
