Case Manager - follow-up and outcomes (Wichita, KS)

Summit Bridge Care Services is looking for a Case Manager. The job centres on follow-up and outcomes for client records, and it would suit someone who likes steady, practical work. You would work closely with colleagues in operations, support and management. What you will do • Report outcomes to funders in clear, short summaries. • Meet clients, understand their needs and build a simple plan. • Follow up regularly and update records accurately. • Connect clients with partner agencies. • Help with applications and appointments for client records. What we are looking for • Strong documentation habits. • Empathy, patience and good boundaries. • Experience in social services, casework or a related field. Nice to have • Bilingual English and Spanish. • Experience with client records. Working arrangement • Full-time, with a steady weekday schedule. • On site in Wichita, with some flexibility after the first few months. • Applications are reviewed on a rolling basis.
