Case Manager - follow-up and outcomes (Virginia Beach, VA)

We are growing our team at Cedar Clear Health and adding a Case Manager whose main responsibility will be follow-up and outcomes. You would join a small organisation of about 27 people based in Virginia Beach. What you will do • Meet clients, understand their needs and build a simple plan. • Follow up regularly and update records accurately. • Report outcomes to funders in clear, short summaries. • Help with applications and appointments for referrals. • Connect clients with partner agencies. What we are looking for • Experience in social services, casework or a related field. • Strong documentation habits. • Empathy, patience and good boundaries. Nice to have • Experience with referrals. • Bilingual English and Spanish. Working arrangement • Part-time, about 24 hours a week, mornings. • On site in Virginia Beach. • Applications are reviewed on a rolling basis.
