Case Manager - follow-up and outcomes (Virginia Beach, VA)

We are growing our team at Ironwood Housing and adding a Case Manager whose main responsibility will be follow-up and outcomes. You would join a large organisation of about 265 people based in Virginia Beach. What you will do • Report outcomes to funders in clear, short summaries. • Connect clients with partner agencies. • Meet clients, understand their needs and build a simple plan. • Follow up regularly and update records accurately. • Help with applications and appointments for maintenance stores. What we are looking for • Empathy, patience and good boundaries. • Experience in social services, casework or a related field. • Strong documentation habits. Nice to have • Bilingual English and Spanish. • Experience with maintenance stores. Working arrangement • Part-time, about 20 hours a week, with flexible days. • On site in Virginia Beach. • We reply to every applicant.
