Case Manager - follow-up and outcomes (Tulsa, OK)

Bright Cedar Health Group is a healthcare provider in Tulsa with clinics and support teams. We are hiring a Case Manager to take on follow-up and outcomes. You would join a large organisation of more than 1,000 people based in Tulsa. What you will do • Meet clients, understand their needs and build a simple plan. • Follow up regularly and update records accurately. • Report outcomes to funders in clear, short summaries. • Help with applications and appointments for clinic supplies. • Connect clients with partner agencies. What we are looking for • Experience in social services, casework or a related field. • Strong documentation habits. • Empathy, patience and good boundaries. Nice to have • Bilingual English and Spanish. • Experience with clinic supplies. Working arrangement • Contract for eight months, with the possibility of becoming permanent. • On site in Tulsa. • We will tell you what to expect at each step of the hiring process.
