Case Manager - follow-up and outcomes (Tulsa, OK)

Fernwood Ember Housing provides affordable homes and support services around Tulsa, and we need a Case Manager to lead our follow-up and outcomes. You would join a large organisation of about 894 people based in Tulsa. What you will do • Report outcomes to funders in clear, short summaries. • Meet clients, understand their needs and build a simple plan. • Connect clients with partner agencies. • Follow up regularly and update records accurately. • Help with applications and appointments for rent assistance. What we are looking for • Empathy, patience and good boundaries. • Strong documentation habits. • Experience in social services, casework or a related field. Nice to have • Experience with rent assistance. • Bilingual English and Spanish. Working arrangement • Full-time, Monday to Friday. • On site in Tulsa. • We reply to every applicant.
