Case Manager - follow-up and outcomes (Tulsa, OK)

Fernwood Housing manages affordable housing communities across Tulsa. We are hiring a Case Manager to take on follow-up and outcomes. Collaboration with local agencies is constant. What you will do • Report outcomes to funders in clear, short summaries. • Connect clients with partner agencies. • Meet clients, understand their needs and build a simple plan. • Help with applications and appointments for waiting lists. • Follow up regularly and update records accurately. What we are looking for • Empathy, patience and good boundaries. • Strong documentation habits. • Experience in social services, casework or a related field. Nice to have • Bilingual English and Spanish. • Experience with waiting lists. Working arrangement • Part-time, about 24 hours a week, mornings. • On site in Tulsa, with some flexibility after the first few months. • We will tell you what to expect at each step of the hiring process.
