Case Manager - follow-up and outcomes (Tucson, AZ)

Silverline Fern Health is a healthcare provider in Tucson with clinics and support teams. We are hiring a Case Manager to take on follow-up and outcomes. You would work closely with colleagues in operations, support and management. What you will do • Help with applications and appointments for electronic records. • Report outcomes to funders in clear, short summaries. • Connect clients with partner agencies. • Follow up regularly and update records accurately. • Meet clients, understand their needs and build a simple plan. What we are looking for • Empathy, patience and good boundaries. • Strong documentation habits. • Experience in social services, casework or a related field. Nice to have • Bilingual English and Spanish. • Experience with electronic records. Working arrangement • Full-time, on a regular day shift. • On site in Tucson. • Applications are reviewed on a rolling basis.
