Case Manager - follow-up and outcomes (Tucson, AZ)

Nova Kestrel Community Partners is a Tucson charity working with residents on basic needs and opportunity, and we need a Case Manager to lead our follow-up and outcomes. You would work closely with colleagues in operations, support and management. What you will do • Help with applications and appointments for donor records. • Connect clients with partner agencies. • Follow up regularly and update records accurately. • Report outcomes to funders in clear, short summaries. • Meet clients, understand their needs and build a simple plan. What we are looking for • Empathy, patience and good boundaries. • Experience in social services, casework or a related field. • Strong documentation habits. Nice to have • Experience with donor records. • Bilingual English and Spanish. Working arrangement • Part-time, about 28 hours a week, afternoons. • On site in Tucson, with some flexibility after the first few months. • We will tell you what to expect at each step of the hiring process.
