Case Manager - follow-up and outcomes (Tucson, AZ)

Bluepeak Iron Community Partners is looking for a Case Manager. The job centres on follow-up and outcomes for grant reporting, and it would suit someone who likes steady, practical work. We are small enough to know everyone's name. What you will do • Meet clients, understand their needs and build a simple plan. • Connect clients with partner agencies. • Report outcomes to funders in clear, short summaries. • Help with applications and appointments for grant reporting. • Follow up regularly and update records accurately. What we are looking for • Strong documentation habits. • Empathy, patience and good boundaries. • Experience in social services, casework or a related field. Nice to have • Bilingual English and Spanish. • Experience with grant reporting. Working arrangement • Full-time, Monday to Friday. • On site in Tucson. • Applications are reviewed on a rolling basis.
