Case Manager - follow-up and outcomes (Toledo, OH)

Bluepeak Pine Community Partners is a Toledo charity working with residents on basic needs and opportunity, and we need a Case Manager to lead our follow-up and outcomes. We are small enough to know everyone's name. What you will do • Meet clients, understand their needs and build a simple plan. • Connect clients with partner agencies. • Help with applications and appointments for food distribution. • Follow up regularly and update records accurately. • Report outcomes to funders in clear, short summaries. What we are looking for • Experience in social services, casework or a related field. • Empathy, patience and good boundaries. • Strong documentation habits. Nice to have • Experience with food distribution. • Bilingual English and Spanish. Working arrangement • Part-time, about 16 hours a week, afternoons. • On site in Toledo, with some flexibility after the first few months. • Applications are reviewed on a rolling basis.
