Case Manager - follow-up and outcomes (Toledo, OH)

Foxglove Wilder Property Management manages apartment communities around Toledo. We are hiring a Case Manager to take on follow-up and outcomes. You would join a small organisation of about 47 people based in Toledo. What you will do • Meet clients, understand their needs and build a simple plan. • Follow up regularly and update records accurately. • Report outcomes to funders in clear, short summaries. • Connect clients with partner agencies. • Help with applications and appointments for rent collection. What we are looking for • Empathy, patience and good boundaries. • Strong documentation habits. • Experience in social services, casework or a related field. Nice to have • Bilingual English and Spanish. • Experience with rent collection. Working arrangement • Part-time, about 28 hours a week, with flexible days. • On site in Toledo. • We will tell you what to expect at each step of the hiring process.
