Case Manager - follow-up and outcomes (Toledo, OH)

Clearpath Vista Housing Group manages affordable housing communities across Toledo, and we need a Case Manager to lead our follow-up and outcomes. You would work closely with colleagues in operations, support and management. What you will do • Meet clients, understand their needs and build a simple plan. • Connect clients with partner agencies. • Help with applications and appointments for repair requests. • Report outcomes to funders in clear, short summaries. • Follow up regularly and update records accurately. What we are looking for • Experience in social services, casework or a related field. • Strong documentation habits. • Empathy, patience and good boundaries. Nice to have • Experience with repair requests. • Bilingual English and Spanish. Working arrangement • Part-time, about 28 hours a week, spread across three days. • On site in Toledo. • Applications are reviewed on a rolling basis.
