Case Manager - follow-up and outcomes (Toledo, OH)

Amberfield Granite Housing provides affordable homes and support services around Toledo, and we need a Case Manager to lead our follow-up and outcomes. You would join a small organisation of about 138 people based in Toledo. What you will do • Help with applications and appointments for lease compliance. • Follow up regularly and update records accurately. • Report outcomes to funders in clear, short summaries. • Meet clients, understand their needs and build a simple plan. • Connect clients with partner agencies. What we are looking for • Strong documentation habits. • Experience in social services, casework or a related field. • Empathy, patience and good boundaries. Nice to have • Bilingual English and Spanish. • Experience with lease compliance. Working arrangement • Contract for eight months, with the possibility of becoming permanent. • On site in Toledo. • We will tell you what to expect at each step of the hiring process.
