Case Manager - follow-up and outcomes (Tampa, FL)

Stonegate Ivory Health Group is a healthcare provider in Tampa with clinics and support teams, and we need a Case Manager to lead our follow-up and outcomes. Clinicians and administrators work side by side. What you will do • Help with applications and appointments for referrals. • Connect clients with partner agencies. • Report outcomes to funders in clear, short summaries. • Meet clients, understand their needs and build a simple plan. • Follow up regularly and update records accurately. What we are looking for • Strong documentation habits. • Empathy, patience and good boundaries. • Experience in social services, casework or a related field. Nice to have • Experience with referrals. • Bilingual English and Spanish. Working arrangement • Part-time, about 24 hours a week, mornings. • On site in Tampa. • We will tell you what to expect at each step of the hiring process.
