Case Manager - follow-up and outcomes (St. Louis, MO)

We are growing our team at Lumen Cobalt Learning Group and adding a Case Manager whose main responsibility will be follow-up and outcomes. Collaboration between teachers and staff is strong. What you will do • Connect clients with partner agencies. • Meet clients, understand their needs and build a simple plan. • Help with applications and appointments for learning materials. • Report outcomes to funders in clear, short summaries. • Follow up regularly and update records accurately. What we are looking for • Experience in social services, casework or a related field. • Strong documentation habits. • Empathy, patience and good boundaries. Nice to have • Bilingual English and Spanish. • Experience with learning materials. Working arrangement • Full-time, with a steady weekday schedule. • On site in St. Louis. • Applications are reviewed on a rolling basis.
