Case Manager - follow-up and outcomes (St. Louis, MO)

We are growing our team at Northwind Quarry Health and adding a Case Manager whose main responsibility will be follow-up and outcomes. Clinicians and administrators work side by side. What you will do • Meet clients, understand their needs and build a simple plan. • Help with applications and appointments for insurance authorisations. • Connect clients with partner agencies. • Report outcomes to funders in clear, short summaries. • Follow up regularly and update records accurately. What we are looking for • Experience in social services, casework or a related field. • Empathy, patience and good boundaries. • Strong documentation habits. Nice to have • Bilingual English and Spanish. • Experience with insurance authorisations. Working arrangement • Part-time, about 28 hours a week, mornings. • On site in St. Louis, with some flexibility after the first few months. • Applications are reviewed on a rolling basis.
