Case Manager - follow-up and outcomes (St. Louis, MO)

Bluepeak Field Housing is looking for a Case Manager. The job centres on follow-up and outcomes for maintenance stores, and it would suit someone who likes steady, practical work. What you will do • Help with applications and appointments for maintenance stores. • Connect clients with partner agencies. • Meet clients, understand their needs and build a simple plan. • Report outcomes to funders in clear, short summaries. • Follow up regularly and update records accurately. What we are looking for • Experience in social services, casework or a related field. • Empathy, patience and good boundaries. • Strong documentation habits. Nice to have • Bilingual English and Spanish. • Experience with maintenance stores. Working arrangement • Contract for nine months, with a review at the end and a chance of extension. • On site in St. Louis, with some flexibility after the first few months. • We will tell you what to expect at each step of the hiring process.
