Case Manager - follow-up and outcomes (St. Louis, MO)

We are growing our team at Harborlight Nest Housing and adding a Case Manager whose main responsibility will be follow-up and outcomes. We value patience and follow-through. What you will do • Meet clients, understand their needs and build a simple plan. • Follow up regularly and update records accurately. • Help with applications and appointments for maintenance stores. • Report outcomes to funders in clear, short summaries. • Connect clients with partner agencies. What we are looking for • Experience in social services, casework or a related field. • Empathy, patience and good boundaries. • Strong documentation habits. Nice to have • Experience with maintenance stores. • Bilingual English and Spanish. Working arrangement • Part-time, about 24 hours a week, mornings. • On site in St. Louis. • We will tell you what to expect at each step of the hiring process.
