Case Manager - follow-up and outcomes (St. Louis, MO)

We are growing our team at Fernwood Rowan Civic Services and adding a Case Manager whose main responsibility will be follow-up and outcomes. Public service means patience, paperwork and pride in the outcome. What you will do • Follow up regularly and update records accurately. • Report outcomes to funders in clear, short summaries. • Connect clients with partner agencies. • Meet clients, understand their needs and build a simple plan. • Help with applications and appointments for procurement. What we are looking for • Strong documentation habits. • Empathy, patience and good boundaries. • Experience in social services, casework or a related field. Nice to have • Experience with procurement. • Bilingual English and Spanish. Working arrangement • Contract for six months, with a review at the end and a chance of extension. • On site in St. Louis, with some flexibility after the first few months. • Applications are reviewed on a rolling basis.
