Case Manager - follow-up and outcomes (St. Louis, MO)

We are growing our team at Fernwood River Learning Group and adding a Case Manager whose main responsibility will be follow-up and outcomes. You would join a large organisation of about 626 people based in St. Louis. What you will do • Meet clients, understand their needs and build a simple plan. • Report outcomes to funders in clear, short summaries. • Follow up regularly and update records accurately. • Connect clients with partner agencies. • Help with applications and appointments for attendance records. What we are looking for • Empathy, patience and good boundaries. • Strong documentation habits. • Experience in social services, casework or a related field. Nice to have • Bilingual English and Spanish. • Experience with attendance records.
