Case Manager - follow-up and outcomes (Spokane, WA)

Bluepeak Yarrow Care Services is a social-care provider serving older adults and people with disabilities around Spokane. We are hiring a Case Manager to take on follow-up and outcomes. You would work closely with colleagues in operations, support and management. What you will do • Connect clients with partner agencies. • Follow up regularly and update records accurately. • Meet clients, understand their needs and build a simple plan. • Report outcomes to funders in clear, short summaries. • Help with applications and appointments for client records. What we are looking for • Strong documentation habits. • Experience in social services, casework or a related field. • Empathy, patience and good boundaries. Nice to have • Bilingual English and Spanish. • Experience with client records. Working arrangement • Part-time, about 28 hours a week, spread across three days. • On site in Spokane. • Applications are reviewed on a rolling basis.
