Case Manager - follow-up and outcomes (Spokane, WA)

We are growing our team at Rivermark Yarrow Civic Services Group and adding a Case Manager whose main responsibility will be follow-up and outcomes. You would join a small organisation of about 24 people based in Spokane. What you will do • Follow up regularly and update records accurately. • Help with applications and appointments for case files. • Meet clients, understand their needs and build a simple plan. • Report outcomes to funders in clear, short summaries. • Connect clients with partner agencies. What we are looking for • Experience in social services, casework or a related field. • Strong documentation habits. • Empathy, patience and good boundaries. Nice to have • Bilingual English and Spanish. • Experience with case files. Working arrangement • Full-time, with a steady weekday schedule. • On site in Spokane. • Applications are reviewed on a rolling basis.
