Case Manager - follow-up and outcomes (Seattle, WA)

Stonegate Summit Community Partners is a nonprofit in Seattle supporting families with food, housing and job help. We are hiring a Case Manager to take on follow-up and outcomes. Flexibility and kindness are expected. What you will do • Follow up regularly and update records accurately. • Help with applications and appointments for grant reporting. • Meet clients, understand their needs and build a simple plan. • Report outcomes to funders in clear, short summaries. • Connect clients with partner agencies. What we are looking for • Strong documentation habits. • Empathy, patience and good boundaries. • Experience in social services, casework or a related field. Nice to have • Bilingual English and Spanish. • Experience with grant reporting. Working arrangement • Full-time, on a regular day shift. • On site in Seattle. • We reply to every applicant.
