Case Manager - follow-up and outcomes (Savannah, GA)

Cedar Nest Early Learning is an early-learning provider in Savannah. We are hiring a Case Manager to take on follow-up and outcomes. Low staff turnover is a goal we take seriously. What you will do • Help with applications and appointments for family communication. • Connect clients with partner agencies. • Meet clients, understand their needs and build a simple plan. • Follow up regularly and update records accurately. • Report outcomes to funders in clear, short summaries. What we are looking for • Experience in social services, casework or a related field. • Empathy, patience and good boundaries. • Strong documentation habits. Nice to have • Bilingual English and Spanish. • Experience with family communication. Working arrangement • Full-time, Monday to Friday. • On site in Savannah, with some flexibility after the first few months. • Applications are reviewed on a rolling basis.
