Case Manager - follow-up and outcomes (Savannah, GA)

Greenridge Pine Community Partners runs community programmes across Savannah, and we need a Case Manager to lead our follow-up and outcomes. Flexibility and kindness are expected. What you will do • Follow up regularly and update records accurately. • Report outcomes to funders in clear, short summaries. • Connect clients with partner agencies. • Meet clients, understand their needs and build a simple plan. • Help with applications and appointments for grant reporting. What we are looking for • Experience in social services, casework or a related field. • Strong documentation habits. • Empathy, patience and good boundaries. Nice to have • Bilingual English and Spanish. • Experience with grant reporting. Working arrangement • Contract for three months, with clear deliverables from the first week. • On site in Savannah. • Applications are reviewed on a rolling basis.
