Case Manager - follow-up and outcomes (San Jose, CA)

Harborlight Quill Health provides primary and specialty care in San Jose, and we need a Case Manager to lead our follow-up and outcomes. You would work closely with colleagues in operations, support and management. What you will do • Follow up regularly and update records accurately. • Meet clients, understand their needs and build a simple plan. • Connect clients with partner agencies. • Help with applications and appointments for patient education. • Report outcomes to funders in clear, short summaries. What we are looking for • Experience in social services, casework or a related field. • Empathy, patience and good boundaries. • Strong documentation habits. Nice to have • Bilingual English and Spanish. • Experience with patient education. Working arrangement • Contract for four months, with a review at the end and a chance of extension. • On site in San Jose. • We reply to every applicant.
