Case Manager - follow-up and outcomes (San Jose, CA)

Greenridge Oak Health is a healthcare provider in San Jose with clinics and support teams, and we need a Case Manager to lead our follow-up and outcomes. We are busy and kind, and we look after each other. What you will do • Meet clients, understand their needs and build a simple plan. • Report outcomes to funders in clear, short summaries. • Follow up regularly and update records accurately. • Connect clients with partner agencies. • Help with applications and appointments for electronic records. What we are looking for • Empathy, patience and good boundaries. • Strong documentation habits. • Experience in social services, casework or a related field. Nice to have • Bilingual English and Spanish. • Experience with electronic records. Working arrangement • Contract for four months, with the possibility of becoming permanent. • On site in San Jose, with some flexibility after the first few months. • We reply to every applicant.
