Case Manager - follow-up and outcomes (San Jose, CA)

We are growing our team at Greenridge Wilder Learning Group and adding a Case Manager whose main responsibility will be follow-up and outcomes. You would join a small organisation of about 101 people based in San Jose. What you will do • Meet clients, understand their needs and build a simple plan. • Help with applications and appointments for learning materials. • Report outcomes to funders in clear, short summaries. • Follow up regularly and update records accurately. • Connect clients with partner agencies. What we are looking for • Experience in social services, casework or a related field. • Empathy, patience and good boundaries. • Strong documentation habits. Nice to have • Bilingual English and Spanish. • Experience with learning materials. Working arrangement • Full-time, with a steady weekday schedule. • On site in San Jose. • We will tell you what to expect at each step of the hiring process.
