Case Manager - follow-up and outcomes (San Jose, CA)

Stonegate Oak Care Services provides in-home and community support in San Jose. We are hiring a Case Manager to take on follow-up and outcomes. The work is personal and so is the culture. What you will do • Help with applications and appointments for care plans. • Follow up regularly and update records accurately. • Connect clients with partner agencies. • Report outcomes to funders in clear, short summaries. • Meet clients, understand their needs and build a simple plan. What we are looking for • Experience in social services, casework or a related field. • Strong documentation habits. • Empathy, patience and good boundaries. Nice to have • Bilingual English and Spanish. • Experience with care plans. Working arrangement • Contract for twelve months, with the possibility of becoming permanent. • On site in San Jose, with some flexibility after the first few months. • We will tell you what to expect at each step of the hiring process.
