Case Manager - follow-up and outcomes (San Jose, CA)

We are growing our team at Foxglove Keystone Civic Services and adding a Case Manager whose main responsibility will be follow-up and outcomes. Benefits and stability attract long tenure. What you will do • Meet clients, understand their needs and build a simple plan. • Report outcomes to funders in clear, short summaries. • Help with applications and appointments for case files. • Follow up regularly and update records accurately. • Connect clients with partner agencies. What we are looking for • Strong documentation habits. • Empathy, patience and good boundaries. • Experience in social services, casework or a related field. Nice to have • Bilingual English and Spanish. • Experience with case files. Working arrangement • Part-time, about 24 hours a week, with flexible days. • On site in San Jose, with some flexibility after the first few months. • Applications are reviewed on a rolling basis.
