Case Manager - follow-up and outcomes (San Jose, CA)

We are growing our team at Foxglove Meadow Community Partners and adding a Case Manager whose main responsibility will be follow-up and outcomes. Mission matters here, and so does looking after staff. What you will do • Report outcomes to funders in clear, short summaries. • Connect clients with partner agencies. • Follow up regularly and update records accurately. • Help with applications and appointments for donor records. • Meet clients, understand their needs and build a simple plan. What we are looking for • Experience in social services, casework or a related field. • Empathy, patience and good boundaries. • Strong documentation habits. Nice to have • Experience with donor records. • Bilingual English and Spanish. Working arrangement • Full-time, on a regular day shift. • On site in San Jose. • We will tell you what to expect at each step of the hiring process.
