Case Manager - follow-up and outcomes (San Jose, CA)

Westbrook Oak Health Group is looking for a Case Manager. The job centres on follow-up and outcomes for patient education, and it would suit someone who likes steady, practical work. Continuing education is supported. What you will do • Meet clients, understand their needs and build a simple plan. • Help with applications and appointments for patient education. • Follow up regularly and update records accurately. • Connect clients with partner agencies. • Report outcomes to funders in clear, short summaries. What we are looking for • Experience in social services, casework or a related field. • Strong documentation habits. • Empathy, patience and good boundaries. Nice to have • Bilingual English and Spanish. • Experience with patient education. Working arrangement • Contract for twelve months, with the possibility of becoming permanent. • On site in San Jose, with some flexibility after the first few months. • Applications are reviewed on a rolling basis.
