Stonegate Haven Early Learning Group is an early-learning provider in San Jose. We are hiring a Case Manager to take on follow-up and outcomes. We celebrate small wins and steady routines. What you will do • Follow up regularly and update records accurately. • Report outcomes to funders in clear, short summaries. • Meet clients, understand their needs and build a simple plan. • Connect clients with partner agencies. • Help with applications and appointments for enrolment. What we are looking for • Empathy, patience and good boundaries. • Experience in social services, casework or a related field. • Strong documentation habits. Nice to have • Bilingual English and Spanish. • Experience with enrolment. Working arrangement • Full-time, with a steady weekday schedule. • On site in San Jose. • Applications are reviewed on a rolling basis.
