Case Manager - follow-up and outcomes (San Jose, CA)

Westbrook Thistle Early Learning operates neighbourhood childcare around San Jose. We are hiring a Case Manager to take on follow-up and outcomes. You would work closely with colleagues in operations, support and management. What you will do • Meet clients, understand their needs and build a simple plan. • Connect clients with partner agencies. • Report outcomes to funders in clear, short summaries. • Help with applications and appointments for classroom supplies. • Follow up regularly and update records accurately. What we are looking for • Strong documentation habits. • Experience in social services, casework or a related field. • Empathy, patience and good boundaries. Nice to have • Experience with classroom supplies. • Bilingual English and Spanish. Working arrangement • Part-time, about 20 hours a week, mornings. • On site in San Jose. • We reply to every applicant.
