Case Manager - follow-up and outcomes (San Diego, CA)

We are growing our team at Clearpath Fox Early Learning and adding a Case Manager whose main responsibility will be follow-up and outcomes. We celebrate small wins and steady routines. What you will do • Connect clients with partner agencies. • Follow up regularly and update records accurately. • Help with applications and appointments for licensing records. • Meet clients, understand their needs and build a simple plan. • Report outcomes to funders in clear, short summaries. What we are looking for • Experience in social services, casework or a related field. • Empathy, patience and good boundaries. • Strong documentation habits. Nice to have • Bilingual English and Spanish. • Experience with licensing records. Working arrangement • Contract for six months, with clear deliverables from the first week. • On site in San Diego. • Applications are reviewed on a rolling basis.
