Case Manager - follow-up and outcomes (San Diego, CA)

Greenridge Clear Care Services supports families and individuals in San Diego with personal care and coordination. We are hiring a Case Manager to take on follow-up and outcomes. You would join a small organisation of about 44 people based in San Diego. What you will do • Help with applications and appointments for visit scheduling. • Connect clients with partner agencies. • Follow up regularly and update records accurately. • Meet clients, understand their needs and build a simple plan. • Report outcomes to funders in clear, short summaries. What we are looking for • Strong documentation habits. • Empathy, patience and good boundaries. • Experience in social services, casework or a related field. Nice to have • Bilingual English and Spanish. • Experience with visit scheduling. Working arrangement • Contract for eight months, with clear deliverables from the first week. • On site in San Diego, with some flexibility after the first few months. • We reply to every applicant.
