Case Manager - follow-up and outcomes (San Antonio, TX)

Ironwood Pine Community Partners Associates is looking for a Case Manager. The job centres on follow-up and outcomes for donor records, and it would suit someone who likes steady, practical work. You would join a small organisation of about 33 people based in San Antonio. What you will do • Follow up regularly and update records accurately. • Help with applications and appointments for donor records. • Connect clients with partner agencies. • Report outcomes to funders in clear, short summaries. • Meet clients, understand their needs and build a simple plan. What we are looking for • Empathy, patience and good boundaries. • Experience in social services, casework or a related field. • Strong documentation habits. Nice to have • Experience with donor records. • Bilingual English and Spanish. Working arrangement • Full-time, Monday to Friday. • On site in San Antonio, with some flexibility after the first few months. • We will tell you what to expect at each step of the hiring process.
