Case Manager - follow-up and outcomes (San Antonio, TX)

Fernwood Thistle Learning Group is looking for a Case Manager. The job centres on follow-up and outcomes for learning materials, and it would suit someone who likes steady, practical work. You would join a small organisation of about 33 people based in San Antonio. What you will do • Help with applications and appointments for learning materials. • Meet clients, understand their needs and build a simple plan. • Follow up regularly and update records accurately. • Report outcomes to funders in clear, short summaries. • Connect clients with partner agencies. What we are looking for • Strong documentation habits. • Experience in social services, casework or a related field. • Empathy, patience and good boundaries. Nice to have • Experience with learning materials. • Bilingual English and Spanish. Working arrangement • Part-time, about 16 hours a week, with flexible days. • On site in San Antonio. • We will tell you what to expect at each step of the hiring process.
