Case Manager - follow-up and outcomes (San Antonio, TX)

Foxglove West Care Services supports families and individuals in San Antonio with personal care and coordination. We are hiring a Case Manager to take on follow-up and outcomes. The work is personal and so is the culture. What you will do • Meet clients, understand their needs and build a simple plan. • Connect clients with partner agencies. • Report outcomes to funders in clear, short summaries. • Follow up regularly and update records accurately. • Help with applications and appointments for family communication. What we are looking for • Strong documentation habits. • Empathy, patience and good boundaries. • Experience in social services, casework or a related field. Nice to have • Experience with family communication. • Bilingual English and Spanish. Working arrangement • Part-time, about 16 hours a week, spread across three days. • On site in San Antonio, with some flexibility after the first few months. • We will tell you what to expect at each step of the hiring process.
