Case Manager - follow-up and outcomes (San Antonio, TX)

Lumen Basin Health provides primary and specialty care in San Antonio. We are hiring a Case Manager to take on follow-up and outcomes. You would work closely with colleagues in operations, support and management. What you will do • Connect clients with partner agencies. • Follow up regularly and update records accurately. • Help with applications and appointments for clinic supplies. • Report outcomes to funders in clear, short summaries. • Meet clients, understand their needs and build a simple plan. What we are looking for • Strong documentation habits. • Experience in social services, casework or a related field. • Empathy, patience and good boundaries. Nice to have • Bilingual English and Spanish. • Experience with clinic supplies. Working arrangement • Contract for four months, with a review at the end and a chance of extension. • On site in San Antonio. • We will tell you what to expect at each step of the hiring process.
